Medication Management
Education of Service Provider
Graduate of an accredited school of nursing. State licensed Registered Nurse or Licensed Practical Nurse.
Duties and Responsibilities
The nurse will coordinate an in-home visit with the client to perform the medication management duties. During this visit it is the nurses responsibility to assess and evaluate the client's current medical status and report any significant changes or concerns to the client's physician. The visiting nurse is responsible to the RN case manager and ultimately to the administrator of ADL Supportive Home Care.
Activities May Include:
- Setup Medications for self dispensing
- Medication teaching
- Evaluation of medication effectiveness and potential adverse effects
- Communication with client physician as needed
- Assessment of vital signs
- Overall client assessment
- Order additional refills from pharmacy
- Coordinate care with other staff and caregivers
